When a very productive officer becomes the subject officer of an Internal Affairs investigation for falsifying police / arrest reports, is the State Attorney's Office required to be notified?

No.
He/she is assumed innocent until proven otherwise.
If found guilty it is up to the administrator to notify SAO (or not). FDLE assumes the administrators will do the right thing and will not cover for "the bad guys." If they don't do the right thing, then everybody suffers - just look at Clermont.
